Bus Transportation Fees:
Bus Transportation fees this year are $125/player
 
Please make checks payable directly to CUSD
 
If you did not turn your check in at player registration night,
you must bring it DIRECTLY to Mike Middlebrook
(DO NOT LEAVE CHECKS AT THE FRONT OFFICE DESK)
 
A fee waiver form is available

Athletic Clearance Forms:
 
Prior to attending ANY tryout, each student must complete AND turn in
 ALL four (4) pages of the 2008/2009 Athletic Clearance Forms
DIRECTLY to Mike Middlebrook's Office. 

DO NOT leave completed forms at the front office.
  
 Once you are cleared, you will receive a clearance pass
from Mike Middlebrook authorizing you to attend tryouts.
