Athletic Clearance Forms:
Prior to attending ANY tryout,
each student must complete AND turn in
ALL four (4) pages of the 2008/2009
Athletic Clearance Forms
DIRECTLY to Mike Middlebrook's Office.
DO NOT leave completed forms at the front office.
Once you are cleared, you will receive a clearance pass
from Mike Middlebrook authorizing you to attend tryouts.

Bus Transportation Fees:
The Bus Transportation Fee for the
2008-09 school year is $125/player
Please make checks payable directly to CUSD
If you did not turn your check in
at player registration night,
you must turn it in DIRECTLY to Mike Middlebrook
DO NOT LEAVE CHECKS AT THE FRONT OFFICE DESK
A fee waiver form is available
